The Beacon Plan Terms & Conditions

The Beacon Plan (the “Plan”) offered by Dental Harbor Savannah, LLC (“Dental Harbor”), is not insurance, is not intended to replace insurance, is not a Qualified Health Plan under the Affordable Care Act, and does not offer minimum essential coverage as defined by applicable federal law. The Plan provides discounts for services and programs performed by designated dental care providers. Discounts vary depending upon the service performed. The Plan does not make direct payment to the providers of dental services. Active Plan members are obligated to pay for all dental services but will receive a discount from those services as indicated by the discount schedule provided herein, which may be modified from time to time with or without notice. Eligibility: Plan membership is available to individuals not currently enrolled with any other discount plan or insurance carrier with whom Dental Harbor or its designated dental care providers have a current in-network provider relationship. “Solo” membership is offered for a single individual. “Duo” membership is offered for two (2) individuals residing within the same household. “Family” membership is offered for up to two (2) individuals age 18 or over residing within the same household plus any dependents under the age of 18 residing within the same household plus any dependents under the age of 25 actively enrolled full time in college.

Initial Enrollment: Plan members enroll in the Plan by submitting a membership application and paying the annual membership fee designated for their desired level of coverage. Members are not eligible for discounts under their Plan until applicable membership fees are paid in full. Dental Harbor may waive, reduce, or modify membership fees for individual members or prospective members, groups of members or prospective members, or all Plan members or prospective members at its sole discretion without notice. A member’s eligibility for discounts under the Plan will begin on the date applicable membership fees are paid in full or as otherwise designated by Dental Harbor (the “Effective Date”).

Renewal: The Plan will continue for a period of twelve (12) months from the Effective Date. Memberships will renew automatically at the end of each period at the then current membership fee for an additional period of twelve (12) months. Members authorize Dental Harbor to charge their credit card, debit card, or checking account on file for such membership fees. In the event a member’s credit or debit card on file has expired at the time of renewal or if payment cannot otherwise be processed, Dental Harbor will make reasonable efforts to contact the member to obtain current payment information. If Dental Harbor is unable to reach the member an invoice may be sent for membership fees owed plus a $10 administrative fee.

Cancellation & Refunds: Members have the right to cancel within 30 days of the Effective Date or annual renewal date and receive a full refund of any membership fees paid, less the value of any services rendered to member prior to cancellation and any processing or administrative fees applicable.

Refunds will be made by Dental Harbor no later than 30 days following receipt of a written cancellation request including a request for refund. Such requests must include the names, addresses, and telephone numbers for all members for whom cancellation of membership is desired and should be addressed to: Dental Harbor, ATTN: The Beacon Plan Administrator, 50 Meeting Street, Suite B, Savannah, GA 31411.

Termination: Dental Harbor may terminate members of the Plan or discontinue the Plan entirely upon written notice. If the Plan or a membership is cancelled, impacted members will receive a refund of any membership fees paid prorated in consideration of the portion of the then current Plan period remaining.

Changes in Membership Levels: Members may upgrade their membership level (for example, from “Solo” to “Duo” or from “Duo” to “Family”) upon request, completion of a new membership application, and payment of applicable membership fees prorated in consideration of the portion of the then current Plan period remaining. No membership may be downgraded (for example, from “Duo” to “Solo” or “Family” to “Duo”) except in the event of death of a member. No substitutions of members will be permitted.

Limitations, Exclusions, and Exceptions: This Plan is a discount membership program offered by Dental Harbor. Dental Harbor is not a licensed-insurer, health maintenance organization, or other underwriter of health care services. No portion of any provider’s fees will be reimbursed or otherwise paid by Dental Harbor under the Plan. Members will receive discounts for services at designated dental care providers. Discounts provided under the Plan are for services or programs rendered on or after a member’s effective date and prior to cancellation or termination of membership. Members are obligated to pay for all dental care services at the time of service. Savings are based upon the provider’s normal fees. Actual savings may vary depending upon specific services or programs purchased. Members should verify such services with each individual provider. The Plan’s discounts may not be used in conjunction with any other discount plan or insurance. All listed or quoted prices are current prices by participating providers and subject to change without notice. Any procedures performed by a non-participating provider will not be discounted. From time to time, certain providers may offer services to the general public at prices lower than the discounted prices available through this Plan. In such an event, members will be charged the lowest applicable price. Discounts on professional services are void where prohibited by law. This Plan does not provide discounts for all procedures. Providers are subject to change without notice. It is the member’s responsibility to verify that their provider participates in the Plan. At any time, Dental Harbor may substitute a provider at its sole discretion. Dental Harbor cannot guarantee the continued participation of any provider. If a provider leaves the Plan, members will need to select another provider. Providers employed or contracted by Dental Harbor are solely responsible for the professional advice and treatment rendered to members and Dental Harbor disclaims any liability with respect to such matters. Complaint Procedure: Members who wish to file a complaint regarding their plan membership must submit such complaints in writing to: Dental Harbor, ATTN: The Beacon Plan Administrator, 50 Meeting Street, Suite B, Savannah, GA 31411. Members have the right to request an appeal if they are dissatisfied with the complaint resolution offered.
